Research abstract

Combat Casualty Care (CCC) continues to face a deadly challenge: the inability to detect internal bleeding and physiologic deterioration early enough to enable timely, life-saving intervention.  According to the Joint Trauma System, uncontrolled hemorrhage accounts for nearly 90% of potentially survivable battlefield deaths, making it the leading cause of preventable mortality in combat. Further, 87% of combat fatalities occur before the casualty reaches a medical treatment facility, underscoring the urgent need for rapid assessment and advanced physiologic monitoring in prehospital and en route care environments. Traditional vital signs—such as blood pressure (BP), heart rate (HR), and oxygen saturation (SpO?)—have long been the foundation of trauma triage, yet they are lagging indicators of hemodynamic collapse. In combat scenarios like blast events, bodily compensatory mechanisms can maintain normal blood pressure despite significant blood loss, giving medics a false sense of stability and delaying urgent intervention. These limitations are particularly dangerous in Tactical Combat Casualty Care (TCCC) settings, where early detection of shock is essential for accurate triage, targeted resuscitation, and survivability during prolonged field and aeromedical evacuation. Flosonics’ FDA-cleared FloPatch, a wireless, wearable Doppler ultrasound sensor, was developed to address this exact gap. FloPatch enables continuous, real-time monitoring of central blood flow, offering a clearer and earlier view of physiologic compromise than conventional vital signs. Data from a hemorrhage model co-developed by Flosonics and the Mayo Clinic demonstrates that blood flow parameters like stroke volume (SV) and flow time corrected (FTc) decline significantly before blood pressure (MAP) drops—proving FloPatch’s value in identifying early-stage shock. The core innovation of this proposal is FloPredict—an AI-powered trauma severity score that elevates the proven clinical foundation of FloPatch to a new level of decision support. While FloPatch already delivers unparalleled real-time visibility into central blood flow via Doppler ultrasound, FloPredict takes the next evolutionary step in vital sign technology by applying Trusted AI and Autonomy to transform raw physiologic data into predictive insights. This enhancement is specifically designed to address the demands of high-stakes environments like TCCC and en route care.
